JOB DESCRIPTION

 

Job Title:  Physical Therapist

Department:  Rehab 

Reports To: Chief Nursing Officer

Status: Part-Time

Date: March 12,2021  

             

Primary Purpose:  The Physical Therapist is responsible for providing quality physical therapy services to patients and residents.  This includes assessment, treatment planning and implementation, education and instruction and documentation of the purpose of assisting patients and residents achieve optimal function and quality of life in an interdisciplinary setting.

 

Essential Duties and Responsibilities:

  • Provides quality patient care by responding through timely screening and evaluation of patients.

  • Establish short-term and long term goals and a treatment plan for each patient in accordance with patient, family and team objectives.   Update and modify the treatment plan according to patient status.

  • Implement therapy services as outlined in the plan of care.   

  • Reports on the condition of the patient to the appropriate staff member and complete timely documentation in the medical record.

  • Instruct family or other team members in the patient’s specific therapy program.

  • Confer with members of the interdisciplinary team both informally and formally at family and team conferences to coordinate the patient’s overall plan of care.

  • Monitor the patient during all phases of treatment.

  • Participate in discharge planning including the development of restorative nursing and/or functional maintenance programs.

  • Supervise therapy services provided by Physical Therapy Assistants and/or rehab aides or technicians. 

  • Complies with all hospital and departmental policies and procedures, safety program requirements, corporate compliance program standards of conduct, all applicable statutes, rules and regulations.

  • Maintains skills and knowledge through participation in educational programs, reading literature, attending workshops, and maintains affiliation with profession’s organization if applicable

  • Performs other duties as assigned. 

 

Physical Demands:

  • Ability to exert up to 50 pounds of force to move objects, equipment and patients served.

  • Ability to regularly stand, walk, sit, stop, kneel, bend or crouch to assist patients. 

  • Ability to use hands to manipulate tools, equipment or controls; reach with hands and arms, balance and lift to assist in patient transfers.

 

Mental Demands:

  • High attention to detail, concentration and alertness levels 

Communication Demands:

  • Ability to communicate (written, verbal, etc.) clearly and effectively in English. 

  • Be approachable; relate well to others; be receptive to feedback and articulate clearly.

  • Speaks clearly and persuasively in positive or negative situations.

 

Education/Training:

  • Bachelor’s degree in Physical Therapy required

  • Current uncumbered state licensure as Physical Therapist. 

Experience:

  • Minimum one (1) year of physical therapy experience preferred.
