Job Title: Social Worker

Status: Full-Time, Non-bargaining, Non-exempt

Primary Purpose: Provides Social Services ensuring the psychosocial needs of the patient population are met. Assess patient needs to access available resources. Serves all age groups of patients, from both inpatient and outpatient.

 

Essential Duties and Responsibilities:

  • Conducts initial psychosocial assessment of patient’s situation to determine needs and goals.

  • Research and advocate for appropriate assistance and resources for patients and/or families.

  • Completes patient consent forms upon admission, including patient rights & responsibilities, advance healthcare directive, and patient disclosure forms.

  • Educates and assists patients and/or families with important and legal forms, such as advance healthcare directives, surrogate, power of attorney, and guardianship.

  • Coordinating discharge planning and maintaining ongoing contact with patients and/or families, outpatient agencies & providers to ensure a safe and/or patient-centered discharge plan.

  • Coordinates case management for patients’ post-discharge needs, collaborating care with insurance, agencies, and families for continued care.

  • Assesses eligibility and assists patients and/or family and outpatients to complete applications for state and public assistance as appropriate. Investigates other financial resources available to the family.

  • Assists with end-of-life care transition and/or the arrangement for funeral or burial of deceased patients, PRN.

  • Participates in skilled nursing interdisciplinary meetings weekly and as needed. Communicates with patient’s care team to ensure a cohesive discharge plan.

  • Co-facilitates the admitting process of skilled nursing patients by gathering information and completing a thorough assessment of patient needs for appropriate admission to KMC.

  • Screen applicants according to the criteria set forth by KMC’s mission statement and Nursing guidelines.

  • Obtains approvals from Business Office, Pharmacy, and Physician/Nursing Admin on an as-needed basis. Initiates pre-registration through Business Office.

  • Communicates daily bed availability with all referring hospitals and facilities on an as-needed basis.

  • Demonstrates the knowledge and skills necessary to provide appropriate care to any age-related needs of the patients served.

  • Complies with all hospital and departmental policies and procedures, safety program requirements, corporate compliance program standards of conduct, all applicable statutes, rules, and regulations.

  • Actively participates in ongoing training as needed to enhance professional growth and development and maintains certification standards and knowledge.

  • Other duties as assigned.

Physical Demands:

  • Prolonged sitting with frequent standing, walking, occasional climbing or balancing, stooping, kneeling, crouching, and crawling.

  • Pushing, pulling, lifting, and carrying up to 10 lbs.

 

Mental Demands:

  • Must have imagination and creative ability, sympathetic approach to patient care.

  • Able to exercise independent judgment.

  • High attention to detail, concentration, and alertness levels needed.

 

Communication Demands:

  • Excellent communication and interpersonal skills (verbal, written).

  • Be approachable; relate well to others; be receptive to feedback and articulate clearly.

  • Be able to work with others in a team environment.

 

Skills/Knowledge:

  • Ability to actively listen and provide feedback.

  • High level of self-awareness, empathy, and sensitivity to others.

  • Great organizational skills.

 

Education/Training:

Required:

  • Bachelor’s degree in Social Work.

Preferred:

  • Master’s degree in Social Work.

  • State of Hawaii Social Work license.

  • Three years of healthcare experience.

 

Preferred Experience:

  • Experience in case management.

  • Knowledge of casework process, counseling, medical and public health setting, working knowledge of social and health resources of community and of individual health.
